EMPACT-SPC is committed to working collaboratively with our public and private partners to solve community problems. We have the resources and expertise to address issues of behavioral health, housing, family and children’s services, employment, crisis intervention, and community and cultural education. ADULT PROGRAM

 

La Frontera / EMPACT-SPC’s Adult Program provides behavioral health services to community members ages 18 years and up.  Clinicians provide counseling services, case management, and medication services for general mental health, substance abuse, and serious mental illness (SMI).  Our outpatient program includes individual, couples, family and group counseling.

Substance abuse recovery services include Intensive Outpatient and Standard Outpatient substance abuse groups, and ongoing recovery support groups.  We also offer many supportive services designed to manage depression, anxiety, grief, loss and stress.

The Adult Trauma Program offers individual and group therapy aimed at restoring full functioning in life and reducing the daily impact of past experiences.  Trauma-educated clinicians and staff work with you to achieve your goals around healing.  We also offer a comprehensive Dialectical Behavior Therapy program and Eye Movement Desensitization and Reprocessing (EMDR).

Adult out-patient services are provided at our Tempe, Glendale, City of Maricopa, Apache Junction, and San Tan Valley locations. Services for SMI members are offered at our central Phoenix location.

CHILD AND FAMILY PROGRAM

 

La Frontera / EMPACT-SPC’s Child and Family Program offers services to promote positive behaviors, family relationships, and overall family health and wellness. Services are provided in the home, office and community, and include Counseling and Psychiatry; Group therapy; Behavior Management; Peer Support; Case Management with other family systems; and Health Education and Screening for pregnant and postpartum women and their infant children.

CRISIS SERVICES

 

La Frontera / EMPACT-SPC’s 24-hour Crisis Hotlines are available to callers who are feeling suicidal, find themselves in a crisis, have been a victim of sexual assault or domestic violence, and/or need a referral for services. Face-to-face services are provided by a Mobile Crisis Intervention Team if immediate assistance is deemed necessary. Hotline services are available to all ages and populations within and outside of Arizona.

24-Hour Mobile Crisis Team assessment, intervention, support, and referral services are available at home or in the community. Services are available to all populations and ages within Maricopa County.

Post-crisis services may be provided by Crisis Transition Navigators who provide support and assist with mapping out, enrolling in, and accessing additional needed community resources and follow-up services.

RALLY POINT ARIZONA

 

The Rally Point Arizona Military Navigator Program provides military service members, veterans, and their family members with crisis prevention, intervention, and peer-based support and resource navigation services.
Rally Point Navigators assist individuals and families with mapping out and helping to access needed services. The Navigator also helps with the coordination of these services and follow-up support with the goal of promoting self-sufficiency, resiliency, and lessening the probability of future crisis.

LA FRONTERA EMPACT-SPC’s PREVENTION DEPARTMENT

 

Our Prevention Department works with schools, senior centers, and other community agencies and business organizations to empower individuals across the life span to become healthy, self-sufficient people.

Peer Counseling Program for Older Adults:
EMPACT-SPC provides training and supervision of senior peer counselor volunteers who offer individual and group support to other seniors. Support groups are conducted on topics such as mindfulness, grief, and caregiving. The focus of the program is to meet the emotional needs of older adults who may be experiencing isolation, depression, or hopelessness.

Life Skills Programs for Youth:
EMPACT-SPC provides school-based life skills development for children and adolescents, using evidence-based curriculum which target concepts such as problem-solving, feelings and coping skills, goal-setting, and social skills. Substance use education, including opioid prevention education, is offered to middle and high school students using the evidence-based curriculum, Too Good for Drugs. While developing skills to increase positive decision-making, goal setting, and how to resist peer pressure, the program also educates youth about the negative effects of drug use and the benefits of being drug-free. Parent and staff presentations are offered as well, in addition to community outreach events being held.

Suicide Prevention Programs:
EMPACT-SPC offers classroom presentations to middle and high school youth, using the Signs of Suicide (SOS) evidence-based curriculum.  Youth are educated about warning signs of depression and suicide, and those at risk are referred for behavioral health services. Parents and school staff are trained on how to identify signs of depression and suicide risk in youth, using gatekeeper trainings such as More than Sad or safeTALK.  In addition, suicide prevention presentations are offered to churches, businesses, and other community organizations, to raise awareness about the issue of suicide.

Suicide Postvention Programs:
EMPACT-SPC is a leader in suicide postvention, providing a variety of services to those who have lost a loved one to suicide. These services include Survivors of Suicide (SOS) support groups which are held in-person, as well as virtually, and facilitated by trained volunteers who themselves have been impacted by suicide. Program staff and trained survivor volunteers are available to provide immediate support to new Survivors of Suicide through LOSS (Local Outreach to Suicide Survivors) Teams; first responder partners, at the scene of a suicide, connect family and friends to LOSS Team services so that important resources, support, and hope can be offered to newly bereaved individuals. Also, presentations are given to schools, churches, businesses, and other organizations on the topic of suicide bereavement and postvention.  In addition, annual events, such as the SOS “Hope and Healing” Conference and the Jeremyah Memorial 5K Walk/Run, provide ongoing support for Survivors of Suicide.

TRAUMA HEALING SERVICES

 

La Frontera / EMPACT-SPC’s Trauma Healing Program offers confidential, 24-hour a day services for individuals affected by sexual assault, domestic violence, or hate crimes. Services include a confidential 24-hour hotline; mobile crisis support; individual, couples, family, and group therapy; and case management and advocacy.

Speakers are available to educate groups and organizations on the challenges facing those who suffer the effects of sexual assault and/or domestic violence, and the services that are available to them.

In collaboration with the Gay Lesbian and Straight Education Network (GLSEN), EMPACT offers hotline, crisis intervention, and support services to LGBTIQ youth and families, as well as training for schools and organizations, and connection to other community resources.

RESIDENTIAL SERVICES

 

LaFrontera/EMPACT-SPC’s Rio de Vida program provides residential substance use treatment to community members aged 18+. Services include both group and individual therapy sessions, life skills, relapse prevention and recreational activities with the goal of teaching and assisting individuals to achieve and maintain sobriety and recovery.
